In the hearts of our nation, no one holds more honor and respect than our deceased service members. Our nation and our military are eternally grateful to them — and to their survivors.
The Department of Defense, in its commitment to provide an enduring remembrance and recognition for those service members, launched the Military In Lasting Tribute. This official online memorial is for honorably serving service members who died while on active duty from 1985 to the present. It is the only DOD memorial that includes peacetime deaths.
The memorial hosts a photo gallery for all the honored service members as well as a specific page for each that can be shared, saved or printed. Each service member’s memorial page will include their name, branch of service, rank, date of death and their photo or branch of service seal.
The DOD respects the privacy of service members and surviving family members. Deceased service members will not be added to the memorial without a family member’s permission. Family members who wish to include their loved one on the memorial should submit a memorial request and photo through the Military In Lasting Tribute website at https://tribute.militaryonesource.mil.
Requirements for submitting a name for the memorial
The DOD invites family members to honor their service members through the memorial. The submission process is open to anyone who has the service member’s “Report of Casualty” DD Form 1300. The service member’s information needed for the memorial submission includes the following:
- Name
- Service branch
- Component
- Rank
- Date of death
- A photo, if available
Visit the memorial website to access the submission form or for more information.
Survivor resources and support
The DOD created the Military In Lasting Tribute memorial to honor our deceased service members. It is equally committed to providing enduring support for their families through Military OneSource.
