Special Medical Needs: A family member with special medical needs is one who meets one or more specific criteria. These criteria include having a potentially life-threatening condition or chronic medical or physical conditions requiring follow-up support of specialty care more than once a year. This would include high-risk newborns, someone who has cancer within last five years, or someone with insulin-dependent diabetes. Other conditions include current and chronic mental health conditions; using inpatient or intensive outpatient mental health services within the last five years; or intensive mental health services required at the present time. A diagnosis of asthma or other respiratory-related diagnosis with chronic recurring wheezing or of attention deficit disorder or attention deficit hyperactivity disorder also qualifies as a special medical need. A condition that requires adaptive equipment, such as a wheelchair, orthotics, or hearing aids; or the use of assistive technology devices, such as communication devices; services, and/or environmental or architectural considerations such as housing modifications also meets the conditions. The EFMP enrollment point of contact will assist in determining if a family member meets criteria for EFMP enrollment.
