The E F M P case management process does not involve coordination and follow-up of medical treatments nor does it include counseling services. Families requiring such services will be referred to the appropriate care providers.
Non-clinical Case Management Services: The family support provider collaborates with you to identify resources and supports and, if required, he or she can help you develop a Services Plan that identifies your family's needs and required resources. The goal is to help families get their needs met, learn about their family member's disability, and maintain the family's independence. EFMP case management does not involve coordination and follow-up of medical treatments or counseling services.
