Text reads: Sponsors must complete their portion of the A F 1466 specifying which family members are requesting travel, and what medical or educational concerns are already known. An A F Form 1466D is required for ALL family members over the age of two traveling OCONUS and for all E F M P identified family members over the age of two traveling CONUS. This will be completed by your dental provider. The D D Form 2792 (Family Member Medical Summary), and D D Form 2792-1 (Special Educational/Early Intervention Summary) demographics are completed by the sponsor and the remainder of the form is completed by a qualified medical or educational professional. The sponsor obtains copies of any Individualized Family Service Plan (I F S P), Individualized Education Program (I E P), or 504 as applicable. An appointment with a P C M or specialist may be needed.
