Exceptional Family Member Program Family Support Reference Guide This guide provides EFMP Family Support staff a clear understanding of the program’s services that provide assistance to military families with special needs.

Guide for Adults with Special Needs This guide provides information and resources to address a wide range of topics related to the concerns of adults with special needs, whether they are adult children or adults diagnosed with a condition or disability.

Special Needs Parent Toolkit – Birth to 18 This toolkit has comprehensive information and tools that are geared towards helping military families with special needs children navigate the maze of medical and special education services, community support and entitlements. The toolkit is broken down into six colorful modules that can be easily downloaded and printed.

EFMP forms

DD Form 2792, “Family Member Medical Summary,” Jan. 2021 This form is used to document a family member’s special medical needs and for enrollment in the Exceptional Family Member Program. This information assists military assignment personnel in matching the family member’s special medical needs against the availability of medical services at the projected duty station.

DD FORM 2792-1, “Special Education/Early Intervention Summary,” Jan. 2021 This form is used to document the special education needs of a child with a disability (birth through 21) and for enrollment in the Exceptional Family Member Program. This information will assist military assignment personnel in matching the child’s special education needs against the availability of educational services at the projected duty station.

Service locators

Eldercare Locator A public service of the United States Administration on Aging, the Eldercare Locator connects older Americans and their caregivers with sources of information on senior services. The service links those who need assistance with state and local area agencies, as well as community-based organizations that serve older adults and their caregivers.

Social Security Office Locator Provided by the Social Security Administration, this locator provides contact information, office hours, maps and driving directions for local Social Security offices, as well as links to other local agencies that can assist individuals with welfare, housing, meal assistance and other support services.

Benefits eligibility checklists

The Benefit Eligibility Screening Tool Provided by the Social Security Administration, BEST is a tool to determine whether an individual could be eligible for benefits from any of the programs Social Security administers. This tool provides eligibility information based on answers given to the questions on the site.

Benefits.gov Benefits.gov provides citizens with easy online access to government benefit and assistance programs. The site’s core function is the eligibility prescreening questionnaire or “Benefit Finder.” Answers to the questionnaire are used to evaluate a visitor’s situation and compare it with the eligibility criteria for more than 1,000 federally-funded benefit and assistance programs. Each program description provides citizens with the next steps to apply for any benefit program of interest.

Education directories

The DoD Directory of Early Intervention, Special Education and Related Services in outside the continental United States Communities The military medical departments, through their educational and developmental intervention services, provide early intervention services and related services in outside the continental United States locations where the Department of Defense school is responsible for educational services. The OCONUS directory is intended to assist the medical and educational assignment coordinators in identifying those military communities in OCONUS locations with pre-established programs or services for children with special needs. The directory does not identify pre-established programs or services for special medical needs.

Education Directory for Children With Special Needs – This DoD directory provides useful information for military families about national and local resources for early intervention (infant and toddlers) and special education (school-aged children) services. The directory provides suggestions and resources to assist families during the educational transition of their child with a disability and provides a profile of and contact information for early intervention agencies and school districts that serve military installations.

EFMP & Me Navigating military and community resources during major life events can be challenging as a military family. The EFMP & Me tool provides checklists, resources and referrals to guide you through each process and find support from the Exceptional Family Member Program.

Through EFMP & Me, users can also contact an EFMP Family Support provider or can learn more about how the program can help you and your family.

MilitaryINSTALLATIONS This site is a resource that provides contact information for programs and services, maps and directions, links to comprehensive location overviews and community points of interest for military installations worldwide. Family Support Centers located on military installations, both in the continental United States and overseas, provide a wide variety of support services for families with special needs members, including enrollment and support for those already enrolled in the EFMP. Contact information for installation services can be found under the program/services titled “Educational and Developmental Intervention Services,” “EFMP-Enrollment” and “EFMP-Family Support.”
