We Want to Hear From You
We are inviting service members with families enrolled in EFMP to participate in the first-ever DOD-wide EFMP Survey.
Conducted by the Office of People Analytics within the Defense Personnel Analytics Center, the survey will focus on the three components of the program: identification and enrollment, assignment coordination and family support. Participation is voluntary, and your individual responses will be kept confidential.
This is your chance to create meaningful change for your family and the military community. Your feedback will allow us to build upon what is working well, address concerns and ultimately enhance EFMP for everyone.
Can Family Members Take The Survey?
While the EFMP survey invitation will be sent to service members with a family member enrolled in EFMP, we encourage spouses and other family members to assist service members in providing their family’s perspective of the program. We want to know the experiences of the whole family!
If you received an email invitation:
Service members with a family member enrolled in EFMP will receive an email from OPA that contains a unique ticket number for accessing the survey.
The survey can be taken on a computer, tablet or smartphone and takes about 15 minutes to complete. You can save your progress at any time and return later.
Getting to the survey:
- Receive the email invitation from OPA
- Follow the link to the survey site
- Enter your unique ticket number
- Start the survey to share your family’s experience
If you did not receive an email invitation:
If you are a service member with family enrolled in EFMP and did not receive an email invitation, you can still access the survey portal
Getting to the survey:
- Follow the prompt for “Don’t have a Ticket Number?”
- Enter DOD ID Number and Day of Birth
- Click “Get my Ticket Number”
